Human-in-the-loop is an operating model—not a button.

Accountable automation requires named decision owners, meaningful evidence and a controlled exception path.

Approval must be meaningful

A human checkpoint adds little value if the reviewer cannot see sources, policy, confidence, alternatives or consequences. The workflow must prepare a decision, not merely request a click.

Design the operating model

  • Name the accountable decision owner.
  • Define what can be recommended and what can be executed.
  • Show the evidence and exceptions required for review.
  • Record approval, modification, rejection and escalation.
  • Measure outcomes and recurring failure patterns.

Match control to risk

Low-risk deterministic actions may be approved for automation. Higher-impact decisions should preserve explicit human authority. The boundary belongs in policy and implementation design.
